AFFORDABLE MUSIC EDUCATION FOR YOUR CHILD

PAYMENT OVERVIEW

Each session consists of 45 minutes of musical fun! Your host is Mr. Ken Shiloh, a master teacher of Longview and other public and Christian schools who specializes in grades K-8 music.

Classes will be held on Tuesday mornings of each week. Classes meet only on days according to the Longview School District calendar (i.e., from September to June, with the same breaks in between). The classes are designed for homeschool children, to give them an opportunity to enjoy music, to perform concerts, and to have a solid education in music: from music theory, to music history, to instrumental music, to polyphonic choral performance: SCMA has it all!

We are presently meeting at the Cowlitz County Fairgrounds.

To gain the maximum benefit for your child, we encourage weekly attendance, as the lessons are sequential. Pay $75 per month (or $18.75 per class) or $250 for the semester (or $16.25 per class).

HOW TO MAKE A PAYMENT

Schedule of Classes:
Lower Elementary (Grades K-1): Tuesdays at 9 AM

Middle Elementary (Grades 2-3): Tuesdays at 10 AM

Upper Elementary (Grades 4-5): 11 AM

If for whatever reason, you may cancel classes with one full week notice, minus a $25 withdrawal fee.

If you pay at the door, please have exact cash for the one class ($20???). Sign in times are 8:50-9:00 AM for grades K-1, 9:50-10:00 for grades 2-3, and 10:50-11:00 for grades 4-5. Arrive a few minutes early to meet other parents and kids! In order to keep our prices down, we do not have a secretary to admit parents and students who arrive after the class begins. Therefore, the doors close promptly at 9 AM (K-1), 10:00 AM (2-3), and 11:00 (4-5), respectively, with no admittance thereafter! Sorry, no exceptions.

For an online payment, please fill out the form below.

For security, we use the ‘///’ shopping cart, a stock plugin for the approved website company, ‘///’.

Terms of the Contract:

Please read, then sign at the bottom:

1. Services Provided: The Shiloh Christian Music Academy (heretofore referred to as the ‘Academy’ or ‘SCMA’), will provide approximately 45 minutes of classroom instruction (heretofore referred to as the ‘Session’) to be attended by both student and a parent or responsible party (heretofore referred to as the ‘Parent’).

After Mr. Shiloh (heretofore referred to as the ‘Teacher’) greets the parent (or parents) and student at the door, the students will then gather around for a time of music. The goal is the enjoyment of music while developing ‘musical literacy’, as the students will learn how to read vocal and instrumental music, understand music theory, tied together with music history and appreciation, and musical performance culminating in our annual Christmas and Spring concerts.

2. Tuition: Since each classroom session has a limited amount of seating, enrollment is first come, first serve. If a classroom is overbooked, SCMA will make every attempt to notify the Parent prior to the classroom session and provide a full refund. However, if SCMA was not able to notify the Parent in time, the Parent shall receive a refund in the form of a check at the door of the Academy.

The price of tuition is:

1-3 sessions: $20 each

4 or more sessions: $17 each

Feel free to cancel your enrollment at any time. All fees paid will be refunded with a $17 service fee. Cancellations must be given at least one week prior to the session(s) to be refunded. The date of the email will be considered the day of notice to receive a refund.

Since each music lesson is built upon

See our ‘Admissions’ page??????

3. Additional Siblings: Each sibling must be separately enrolled. Due to our extremely low tuition rates, the Academy does not offer discounts for siblings.

4. Academy Rules: [ ] [ ] (Initials). Student’s enrollment at the Academy is subject to the general statements, rules, regulations, conditions, and financial terms contained in the Academy’s website at shilohschool.biz (on the contract page?????), which may be amended from time to time. Parent acknowledges that Parent and Student must abide by such Academy rules and guidelines.

5. Supervision: [ ] [ ] (Initials). Since the nature of homeschooling is a parent/guardian working together with their student, a Parent or other responsible party (must be 18 years or older) is required to attend each session. For example, some parents may choose to ‘trade off’ watching each other’s Student during a session (please inform Mr. Shiloh at the beginning of the Session if there are more than 3 students under your supervision). It is understood that the Academy shall not be liable if the Parent chooses another guardian to accompany their child to a session. However, if your child has a history of challenges in following classroom rules, please accompany your child to each session.

It is understood that SCMA is free from all liability in Parent’s choosing of supervision of their child and is neither encouraging nor discouraging such supervision. During each Session, the student must be supervised by an adult who is at least 18 years. Students may not, under any circumstances, be left without parental or parent-approved supervision. Parents who leave their child without an adult to supervise will be permanently terminated from attending the Academy.

Classroom Discipline Policy: If a child is disrupting a class, Mr. Shiloh will politely address the infraction. If the child needs extra assistance, they will be excused to the back of the classroom to fill out a short behavioral form, together with the Parent. Upon completion, the student may return to class.

_____ (Please initial) I understand the supervision rules of the SCMA.

6. Termination of Student’s Attendance: The Academy reserves the right to refuse service. The Academy also has the right to suspend or terminate this contract if the parent and/or student is significantly abridging the flow of education or in the hindrance of SCMA administrative duties as deemed either ‘chronic’ or ‘severe’ by the Teacher. Other grounds for termination of contract may or may not be expressed in these documents. At the termination of the contract, the customer will receive a full refund for any unattended classes paid for in advance, paid for by check, through the mail or through (Paypal or other payment service TBA).

7. Tardy Policy:

As is the policy at the local public schools, the door will remain locked during teaching time. However, in order to provide service at very reasonable rates, SCMA does not have a secretary to allow entry for late parties. Therefore, a parent and student who arrives late will not be admitted after the designated time of the session.

The doors will be closed and locked at the beginning of each Session. In signing this contract, both parent and student agree not to knock on the door nor create any other disturbance to the class. Moreover, there will be no refund for any missed class (please keep in mind that the low cost of tuition will, in time, more than compensate for the payment of the missed class). Therefore, we highly recommend that the parent and student arrive at least 10 minutes early. Come early and enjoy meeting the other parents and their kids!

_____ (Please initial) By initialing, I have read and agree to the SCMA ‘Tardy Policy’.

(On the inside of the classroom, a sign on the door reads, “For security reasons, please do not open the door without staff approval after the session has begun” and on the outside, “For security reasons, this door may not be opened after a session has begun.”)

8. Photos and Images: The Parent agrees to allow the Student’s photograph, voice, and image to be used by the Academy for use in the Academy’s publications, promotion materials, social networks, and website, without compensation and without prior notice. However, with the exception of incidental identification (e.g., a first name on a name tag), neither the name of the Student nor of the Parent, nor any other personal information, shall be publicly shared. Moreover, Parents are encouraged to take videos and/or make recordings of the Academy’s Christmas and Spring Concerts and other special events.

9. Academy/Family Cooperation: A positive and constructive relationship between the Academy and Family Member (for example, a parent, student, or other person associated with a student) is essential to the Academy’s educational purpose and responsibilities to its students. If any Family Member engages in behavior, communications, or interactions on or off campus, that is disruptive, intimidating, overly aggressive, or reflects a loss of confidence in or disagreement with the Academy’s policies, methods of instruction or discipline, or otherwise interferes with the Academy’s safety procedures, responsibilities, or accomplishment of its educational purpose or program, the Academy reserves the right to dismiss the Family or Family Member from the Academy. The Academy may also place restrictions on a Family Member’s involvement or activity at the Academy for other reasons that the Academy deems appropriate. Any determination under this Paragraph shall be in the Academy’s sole discretion. In the event of a termination of contract, there will be a full refund for all unattended sessions. The Academy also reserves the right to withdraw an offer of enrollment or re-enrollment at any time and to void an executed Enrollment Contract.

10. Medical Authorization: If the Parent is absent from the Academy, and, in the opinion of the Academy, the Student needs medical or surgical services which require Parent’s pre-authorization or consent, Parent hereby authorizes, appoints, and empowers the Academy to act as Parent and furnish such consent on Parent’s behalf. Parent confirms that it is Parent’s desire that the Student be furnished with such medical or surgical services as soon as reasonably possible after the need arises. Parent hereby releases and holds the Academy harmless from any liability which might arise from the giving of such consent. Parent agrees to reimburse the Academy for any medical expenditures made on Student’s behalf. Parent agrees not to hold the Academy liable in any way concerning the toys given as reward. It is incumbent on the Parent to insure the appropriateness and safety of the awarded toys.

11. Promotional Materials/Statements: The Academy continually strives to ensure the accuracy of all written materials, including, but not limited to, promotional information, catalogs, brochures, handbooks, and advertising. In an effort to do so, however, information included in the materials (including class sizes, pricing, etc.) may change as the Academy grows. For the most accurate, up-to-date changes, see the shilohschool.biz website.

12. Governing Law/Waiver of Jury Trial: This Contract and the rights and obligations of the parties shall be governed by and construed in accordance with the laws of the State of Washington without regard to principles of conflicts of law. The parties agree to waive the right to jury trial over any claims pertaining to Student’s enrollment, attendance, or separation from the Academy including, but not limited to, claims of breach of contract, under statute, ordinance, or common law. The exclusive venue for any claim shall be any Washington state court in Cowlitz County, Washington as appropriate.

13. Force Majeure: The Academy’s duties and obligations under this Contract shall be suspended immediately without notice during all periods that the Academyis closed because of force majeure events including, but not limited to, any fire, war, governmental action, epidemic, or any other event beyond the Academy’s control. If such an event occurs, the Academy’s duties and obligations in this Contract will be postponed until such time as the Academy, in its sole discretion, may safely reopen. In the event that the Academy cannot reopen due to an event under this clause, the Academy is under no obligation to refund any portion of the tuition paid.

14. Understanding of Terms: Please read this Contract carefully. By signing below, Parent acknowledges that Parent understands and agrees to the terms of this Contract. If Parent has questions about the terms, Parent is encouraged to seek advice of counsel or to seek clarification from the SCHA administration via email at [email protected].

15. Entire Agreement: This Agreement sets forth the entire understanding of the parties hereto with respect to the subject matter hereof and merges and supersedes all prior and contemporaneous oral understandings between the parties. There have been no representations or warranties made by any party other than the representations and warranties contained herein.

Dear Parent or Guardian,

Thank you for considering the Shiloh Christian Music Academy! As a long-time teacher in the Longview School District, I can say with confidence that the public schools do not typically offer the level of music skills and education of the Academy. These Sessions will provide your child with a musical education that, whether a performing musician or vocalist or not, will be of tremendous benefit over their lifetime. Again, thank you for considering the Shiloh Christian Music Academy!

May the Lord bless you and yours,

Ken Shiloh

ENROLLING YOUR CHILD

AFFORDABLE MUSIC EDUCATION FOR YOUR CHILD

Each session consists of 45 minutes of musical fun! Your host and teacher is Mr. Ken Shiloh, a teacher in the Longview and other public schools, who specializes in grades K-8 music.

How It Works

Classes will be held on Tuesday of each week during the school year. There are three combined classes: Kindergarten and first grade, second and third grades, and a final class for fourth and fifth grades. The classes are designed for homeschool children, to give them an opportunity to enjoy music, to perform concerts, and to have a solid education in music: from theory, to music history, to instrumental music, to polyphonic choral performance.

In order for your child to gain the full benefit of the Shiloh Christian Music Academy, we encourage attendance each week. Therefore, sessions are paid for either by the month or by the semester. Classes are $20 per session if paid by the month (or 4 classes per month for $80) or $15 per class when paid for all classes during either the Fall (14 classes for $210) or Spring (18 classes for $270). These fees include the performance of our annual Christmas and Spring musical concerts. We hope to see you at the Shiloh Christian Music Academy!

Terms of the Contract:

General Information

1. Services Provided: The Shiloh Christian Music Academy (heretofore referred to as the ‘Academy’ or ‘SCMA’), will provide approximately 45 minutes of classroom instruction (heretofore referred to as the ‘Session’) to be attended by both student and a parent or responsible party who is over 18 years of age (heretofore referred to as the ‘Parent’).

To keep our fees low, the SCMA does not have additional staff support other than Mr. Shiloh. Therefore, the Parent should either be in the class or outside in the hallway. Parents leaving their child unattended will result in termination of this contract, with a full refund for any unattended classes. Refunds will be given through Paypal.

Some parents may choose to ‘trade off’ watching each other’s Student during a session (please inform Mr. Shiloh at the beginning of the Session if there are more than 3 students under your supervision). It is understood that the Academy shall not be liable if the Parent chooses another guardian to accompany their child to a session. It should be noted that the Academy neither encourages, nor discourages parents from watching other parents’ students during class time.

After Mr. Shiloh (heretofore referred to as the ‘Teacher’) greets the parent and student at the door, the students will then gather around for a time of music. The goal is music education and the enjoyment of music culminating in our annual Christmas and Spring concerts.

    2. Tuition: Since each classroom session has a limited amount of seating, enrollment is first come, first serve. If a classroom is overbooked, SCMA will make every attempt to notify the Parent prior to the Session. If a student is not able to attend a class because of overbooking, the Parent shall receive a full refund through Paypal.

    Classroom Management: What to expect

    3. Mr. Shiloh has years of teaching experience and has a reputation for solid classroom management. However, the SCMA is not designed to work with children with severe behavior concerns, at least not in the classroom setting. At Mr. Shiloh’s discretion, any severe student, and/or parental, disruption will result in termination of this contract, with a full refund given for unattended classes. For example, the SCMA is not suitable for students with a history of striking other students or students who habitually exhibit very loud and dramatic outbursts in public settings. Thank you for your understanding.

    4. Cancellations: Feel free to cancel your enrollment at any time. There is a $20 cancellation fee and cancellations must be given at least one week prior to the Session(s) to be refunded. The date of the email will be considered the day of notice to receive a refund. At Mr. Shiloh’s discretion, the cancellation fee may be waived if a family hardship (e.g., loss or illness) is involved.

    5. Additional Siblings: Each sibling must be separately enrolled. Due to our low tuition rates, the Academy does not offer discounts for siblings.

At the termination of the contract, the customer will receive a full refund for any unattended classes paid for in advance, paid through Paypal.

    7. Tardy Policy:

Please arrive 15 minutes prior to the beginning of class. Students arriving late are a disruption to the class. Chronic tardiness may result in cancellation of services, with a full refund for unattended classes.

    8. Photos and Images: The Parent agrees to allow the Student’s photograph, voice, and image to be used by the Academy for use in the Academy’s publications, promotion materials, social networks, and website, without compensation and without prior notice.
